Our Practice Charter




We have a duty of confidentiality to all of our patients.  We regard this as being of the utmost importance.

We do not given out information to a third party without the written permission of a patient.  This applies to all information that we hold, medical and non-medical such as telephone numbers, addresses etc.


We will not give information to a husband or wife without specific permission to do so.  This also applies to the affairs of any child who is considered capable of making the decision that he or she does not want information divulged to a parent or guardian.


If you would like to grant permission for your next of kin or other named individual to have access to your medical information consent forms are available from Reception.


Data Protection


The practice is registered under the Data Protection Act 1998.  Our registration no. is Z9869715


Access to Medical Records


You have a right to examine your notes according to the provisions of the Data Protection Act 1998.  Please contact the Practice Manager for further information



Zero Tolerance


This Practice operates a zero tolerance policy—any patient being abusive or violent towards one of the GPs, Health Care Professionals or other members of staff will be removed from the Practice list and reported to the police if necessary


Complaints and Suggestions


We strive to give you the best possible care and attention. If you have any suggestions or are unhappy about any aspect of our service, please ask to see the Practice Manager who will be happy to listen to your suggestions or criticisms.  We have a written complaints procedure for matters that cannot be resolved, complaints should be addressed to the Practice manager.  We will take the matter seriously and do our best to resolve any difficulties following the practice complaints policy.

Don't waste appointments - please let us kown if you cannot attend


Be considerate if there are delays or difficulties.


Do let us know if you change your name, address and or telephone number.
